Why bill agitation separately?
Because it's hours of equipment running before a single load moves. Buried inside a per-load rate it makes your hauling look expensive while hiding where the time actually went.
Loads or gallons?
Whichever your customer thinks in — gallons for liquid systems, loads for solid. Pick one per job, state it clearly, and keep the rate consistent so year-over-year comparisons are easy.
Should distance be a surcharge?
Yes, beyond a stated radius. Hauling to a field five miles further is real time and fuel, and a stated surcharge is far cleaner than quietly inflating the per-load rate.
